A percentage by itself doesn't tell you very much.

Take operating margin.

12%.

Is that strong?
Is it declining?
Is it below the practice's target?
Is something outside the practice affecting it?

You can't answer those questions from the number alone.

For medical practices, meaningful financial analysis often requires comparing the result against history, targets, relevant benchmarks and the conditions surrounding the practice.

A benchmark isn't a prescription.

It's context.

What financial metric would you most want to compare, not just track, in your practice?

A full schedule can make a practice feel healthy.

But activity and financial strength aren’t always the same thing.

Collections can be strong while cash is being absorbed by payroll, overhead, debt, taxes, or outstanding receivables.

That’s why I think one of the more useful questions for a practice owner is:

“Where is the cash actually going?”

Not just how much came in.

What financial question would you ask if you had 30 minutes with your practice’s numbers?

A medical practice can have more revenue, more patients and more activity, and still have a financial problem hiding underneath.

That’s why I don’t think the most useful question is always:

“How much did we make?”

Sometimes it’s:

What happened to the margin?
What changed in overhead?
Did collections keep pace with production?
What did the new physician actually add?
Where is cash being absorbed?

The numbers aren’t just there to report what happened.

They can help explain what is happening inside the practice.

What number would you want to understand better if you were reviewing your practice today?

09/22/2026

A tax payment tells you what was paid.

It doesn’t automatically tell you whether the number still makes sense.

For a physician, the more interesting questions are:

Is the estimate based on current income?

Does it fit the practice’s cash position?

And what does it tell you about the rest of the year?

That’s where tax planning becomes more than compliance.

The number is the starting point. The questions around it are where strategy begins.

What tax question would you want answered before the next deadline?

A busy practice can still have financial blind spots.

More physicians. Higher collections. New equipment. Bigger payroll.

Each decision changes the financial picture, and sometimes the tax picture with it.

The question isn’t just whether your practice is growing. It’s whether your numbers are keeping up with the decisions you’re making.

What part of your practice’s finances would you most like to understand better?

09/01/2026

Your tax bill is only one part of your practice’s financial picture.

Revenue changes. Payroll changes. Compensation changes. Cash gets invested back into the practice. And eventually, taxes need to be paid.

Looking at each piece separately can make the numbers harder to understand. The better question is: how do they work together? That’s where financial visibility becomes useful. What part of your practice’s finances do you wish you had more clarity around?

Quarterly tax payments aren’t a “set it and forget it” number.

For physicians, income can change significantly throughout the year, especially when a practice is growing, compensation changes, or additional income streams come into play.

So the better question isn’t only “Did I make my payment?” It’s “Does my payment still reflect where I am today?”

What usually changes the most for your practice during the year, income, expenses, or both?

08/20/2026

Tax deadlines are easy to find.

What happens before them is where things get interesting.

For physicians and practice owners, income can change, new providers can join, and major investments can shift the numbers long before a tax payment is due. Knowing the deadline is important. Knowing what your numbers are telling you before you get there is better. What part of tax season tends to catch you off guard?

Growth changes the tax picture.

Hiring a physician, expanding a practice, or investing in new equipment can trigger tax implications that aren’t obvious when the decision is made.

The question isn’t just “Can we afford it?”
It’s “What will this decision change financially?”

The smartest practice decisions account for the tax impact before the commitment is made.

As your practice grows, so does the complexity behind the scenes.

Bringing on a new physician, expanding into another location, adding a new income stream, or becoming a practice partner are all signs of success, but they also introduce new financial and tax considerations.

Too often, physicians continue using the same tax strategy even though their practice has changed significantly. That’s where opportunities are missed.

Tax Compliance isn’t just about filing accurately.
It’s about making sure your financial strategy keeps pace with your practice.

The Axion Take: Growth shouldn’t outpace your tax strategy
