Each decision may affect taxable income, future RMDs, and income-related Medicare considerations. That is why the “best” choice cannot usually be determined by looking at one transaction or one year in isolation.

Your 2026 Medicare premium may be based on income reported two years earlier.
The standard Part B premium is $202.90 per month in 2026. Income-related Medicare adjustments may apply when 2024 modified adjusted gross income exceeded $109,000 for an individual or $218,000 for a married couple filing jointly.
That is why retirement tax decisions can have a healthcare-cost effect. A Roth conversion, large realized gain, or retirement-account distribution may increase income used for the premium calculation. On the other hand, a recent retirement, work reduction, or other qualifying life-changing event may support a request for a new IRMAA determination.
The takeaway is not “avoid income.” It is “model the connected decisions before acting.”
