$3.50 in new payroll for every $1 of new revenue. That was the ratio inside an insurance agency that, on the surface, looked like it was doing fine.
Revenue was up 3%, new business was up 22%, and expenses came in under plan in almost every discretionary category.
But employee expenses were up 23% over the same stretch. Operating margin slid from 37% to 28% and net income fell close to 22%. In dollars, the agency added about $54,000 in new revenue and about $188,000 in new payroll.
None of this was reckless. The agency was hiring because new business was growing, and that part was working. The problem was that renewals, which made up 81% of revenue, stayed flat. The new hires cost money from day one, and the revenue that was supposed to carry them was still catching up.
The part that made it easy to miss is that the budget balanced on paper. Lower spending on marketing, lead generation, and a few other categories covered the payroll overage, so the budget said everything was fine while the P&L said otherwise.
We wrote up the full case study on the blog, including the five numbers the owner started tracking after this review. Link is in the first comment.

A CPA had already reviewed the return. We still found $21,000 a year in potential federal tax savings sitting in it.

The client is a captive insurance agent who's been in business almost ten years, with two offices in two buildings. He asked us to take a second look at his 2025 return, and not because he was unhappy with his CPA. He trusted the firm and thought the work was competent.

The issue was growth. The agency had gotten bigger, and one election never got made: his state's pass-through entity tax. On roughly $846,000 in S-corp income, that missed election was the biggest opportunity on the whole return. He had even discussed PTET with his CPA and assumed it was already in place. It wasn't.

Two other things came up in the review: $8,707 in federal underpayment penalties from a year with $208,000 in total tax and only $21 in withholding, and a question about whether his owner compensation matched the work he was doing in the business.

None of this makes the original CPA negligent. It just means the return deserved a second set of eyes.

And that $21,000 is one year. We only looked at 2025. If the same election was missed in the years before it, the real number is a multiple of that.

The full write-up is on the blog, including what we looked at and why each item mattered. Link in the first comment.
