Please be very careful when addressing correspondence from government agencies. Attached is a scam Form 5102 businesses are receiving from the "Annual Records Service" in regards to the requirements for the Mandatory Beneficial Ownership Reporting. It is a fake form. Form 4022 is also being distributed as a scam form as well.

Please visit the following FAQ's regarding the new requirements for Beneficial Ownership Information Reporting (BOI), to see if this applies to your business. If you have a business that is registered with the Sec of State to do business in IL (and many other businesses as well), you will have to file this report by 12/31/24. Any new businesses formed in 2024 or later will have 90 days to complete the BOI report. The fines are substantial for failure to timely file this report, so please be aware. Kevin J. Huffman joined the firm in 1982 and became an owner in 1989. He received his Bachelor of Sciences Degree in Accountancy from the University of Illinois in 1981, and became a Certified Public Accountant in May of 1981. Kevin now runs the firm's office in Arthur. He is also a Registered Investment Advisor Representative.
Kevin provides a variety of financial services to his clients. His clients include families and individuals, farmers, small businesses, local governments, and not-for-profit organizations.
